Welcome To Ambiance Surgical Arts Center!

You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

First Visit

Information to Bring to Your Initial Consultation

To save time and help us develop your treatment plan, please have the following information with you at your initial visit:

Your surgical referral slip
Any x-rays if applicable (note: if your dentist or physician has taken x-rays, we may request that they forward them to our office. If there’s not enough time, you can pick them up from them and bring them with you to the appointment. If additional x-rays are necessary, we can take them at our practice)
A list of medications you’re presently taking
If you have medical or dental insurance, bring the necessary completed forms. This will help us process any claims for you more quickly.

We recommend you fill out the forms and submit them to us online but if printing them is easier, please print these forms and fill out prior to your appointment.

In House Prescriptions

At Ambiance, we offer you medications from our Florida approved in house pharmacy, so you no longer have to wait in long lines or have delays in receiving your postoperative medications.
